It is the policy of this practice to maintain privacy and security of personal health information at all times. This practice will not disclose your personal health information to third parties unless we have your consent or you are at risk of harm without treatment and you are unable to provide consent.

 

As our practice is committed to preventative care your doctor will seek your permission to be included on our recall system.

 

We may issue you with a recall notice from time to time offering you health services appropriate to your care.

 

If you do not wish to be part of this system please let your doctor or the receptionist know.

Your doctor will advise when they expect your results to arrive at the practice. Please call reception to confirm your results are available and make an appointment to review them with your doctor.

My Health Record Ready!

Lynwood Medical Centre is now My Health Record ready. Registering for a My Health Record is completely voluntary and free. If you choose not to have a record it does not limit your ability to access healthcare services.

 

A My Health Record will provide healthcare organisations involved in your care important health information about you such as allergies/reactions, current medications, past medical history and immunisation history. This will be supplied in the first instance by your nominated GP  and be added to over time by providers involved in your care.

 

You can personally control which healthcare organisations can access your My Health Record and you can also place restrictions on access to specific documents within your My Health Record.
